[SUMMARY] We conducted a qualitative descriptive study to depict the adjustment process of fathers for performance of father’s role in caring for preterm infants with high risk of developing cerebral palsy. This study also aimed to develop strategies of nursing support in a NICU. Semi-structured interviews were conducted among five pairs of parents with preterm infants diagnosed with cerebral palsy. Based on the results, we finalized seven themes “ : caring for my wife while communicating with her about childbirth,” “facing the realities of life and praying for the good health of our child,” “caring for my wife irrespective of the NICU environment and/or the child's appearance,” “prioritizing my child over my official work,” “increasing awareness by communicating as a couple and supporting my wife’s parenting efforts,” “taking part in parenting in cooperation with my wife, where I am aware of everyday parenting and household duties and my child’s growth and development,” and “being intentionally involved with siblings of preterm infant while cooperating with my wife and our parents.” The fathers went through three stages based on the characteristics of the adjustment process : the embryonic period, trial and error period, and cooperation expansion period. As part of the nursing support program in the NICU, we focus on the psychological confusion that fathers undergo immediately after the birth of preterm infants. We should also provide support in creating an environment where we empathize by listening to them. After the father and mother have mentally settled down, we should create a space for discussion to establish a common understanding between the parents about the preterm infant.
