[要旨] 本研究は,子宮頸部前がん病変と診断された女性が経験したスティグマとスティグマによる他者との関係や生活への影響を明らかにし,スティグマを予防・軽減するための支援を検討することを目的とした。高度異形成または子宮頸がんの最終診断がついた女性14名を対象に,面接調査,記録調査からデータ収集を行い,質的帰納的に分析した。その結果,子宮頸部前がん病変と診断された女性は,【偏った性交経験へのレッテル】や【がん患者という同情的な視線】を感じ,【過去の行いに対する後悔の念】や【不確かな心配事に対する自責の念】という感情を抱くというスティグマを経験していた。これらのスティグマの経験により【子宮頸がんに対する偏見の予期】や【パートナーとの関係維持における障壁】,【自己開示の躊躇による受診行動の妨げ】,【今後の人生への脅威】となる一方で,【否定的体験から他者貢献への転換】という他者との関係や生活への影響を生じていた。子宮頸部前がん病変と診断された女性に対し,否定的な思考や感情を予防・軽減するための支援,他者との関係や生活上の支障を予防・軽減するための支援,スティグマを跳ね返す強さを獲得するための支援を行う必要性が示唆された。スティグマは社会的なラベルが貼られることで生じるため,がん検診の周知と共にHPVの脅威や子宮頸がんが身近な疾患であることを社会全体に意識付けを図っていくための方略を検討する必要がある。
[SUMMARY] The objective of this study was to elucidate the stigma experienced by women diagnosed with pre-cancerous cervical lesions and the effect this stigma has on their relationships with others and their daily lives, in order to consider types of support that can prevent or alleviate this stigma. The participants were 14 women who received a final diagnosis of severe dysplasia or cervical cancer. Data was collected via direct surveys and examination of medical records, and a qualitative inductive analysis was performed. The results indicated that the participants, who were diagnosed with pre-cancerous cervical lesions, felt “label for biased sexual intercourse experience” and “sympathetic look for a cancer patient” and experienced stigmas that took the form of “regret for one’s past behavior” and “remorse for uncertain worries.” These stigmas caused “anticipation of prejudice against cervical cancer”, “barriers to maintaining relationships with partners”, “impediments to consultation behavior due to hesitation in self disclosure” and “threat to one’s future life.” However, it was also elucidated that the experience of these forms of stigma led them to “convert these negative experiences into work to benefit others.” The results suggest that there is a need to provide the following types of support to women diagnosed with pre-cancerous cervical lesions : support designed to prevent/alleviate negative ideas and feelings, support designed to prevent/alleviate obstacles to their relationships with others and in their daily lives, and support designed to help them alleviate the stigmas. Because stigmas arise as a result of the affixation of social labels, there is a need to investigate strategies designed to inform society as a whole of the importance of cancer testing, the threat of HPV, and the fact that cervical cancer is a common illness.
