本研究の目的は,高度先進医療を提供する病院における専門看護師・認定看護師に対する看護師のニーズを把握し,専門看護師・認定看護師の活動の方向性を検討することである。高度先進医療を提供する大学病院1施設の看護師に自作の質問紙調査を行い,297人から有効回答を得た。その結果,以下が明らかとなった。平成19年4月から平成20年1月までに145人(48.8%)の看護師が,専門看護師/認定看護師に271件の相談を行っており,皮膚・排泄ケア認定看護師と,がん看護に関わる専門看護師・認定看護師に対する相談が多かった。看護師が専門看護師・認定看護師に依頼・相談したい内容は,身体的・心理的苦痛の軽減,終末期ケアなどの「患者・家族ケア」が最も多く,続いて,最新知識の習得などの「医療者を対象とした勉強会の開催」であった。看護師は,高度先進医療を提供する大学病院の特徴を反映した,部署内では解決できない問題を数多く認識していたが,38.7%の看護師が「誰にどのように相談してよいか分からない」,37.4%が「相談してよいかどうかわからない」と回答した。以上より,専門看護師/認定看護師は,相談の依頼を待つだけでなく,様々な機会を利用して,現場でのニーズを敏感に捉える,ニーズに合わせた学習を提供する,複雑で多種多様な問題に的確に対応できるよう専門看護師・認定看護師同士が連携を図る,専門看護師・認定看護師にアクセスしやすい体制を整える,自らが役立てると思う場面に積極的に加わり多職種と連携して問題解決にあたる重要性が示された。
The purpose of the present study was to elucidate nurses' needs for Certified Nurse Specialists(CNSs)/Certified Nurses(CNs)and clarify their roles at a general hospital that provides highly advanced medical treatment. An original questionnaire created by the authors was completed by a total of 297 nurses at the general hospital. The following results were obtained. : From April 2007 to January 2008, 145(48.8%)of the 297 nurses consulted with CNSs/CNs about 271 cases. Nurses reported consulting with CN in Wound, Ostomy, and Continence Nursing and CN in Cancer Pain Management Nursing, CN in Brest Cancer Nursing, and CNS in Cancer Nursing. Most nurses consulted with CNSs/CNs regarding “care for patients and families" with the aim of reducing the physical and psychological pain and when providing terminal care. Nurses also consulted with CNSs/CNs during “study meetings for medical staff" in order to gain up-to-date knowledge. The nurses reported encountering many difficulties that they could not solve independently. Thesedifficulties reflected the unique conditions they faced in a general hospital that provides highly advanced medical treatment. A total of 38.7% of nurses responded, “I can't understand who to consult with and how to consult with them", while 37.4% responded, “I can't decide if my difficulties are problems that warrant consulting with them or not." The present findings indicate that it is necessary for CNSs/CNs to receive consultations, but also proactively address the needs of nurses. Furthermore, CNSs/CNs must provide up-to-date medical/nursing knowledge and collaborate with CNSs and CNs in orderto solve difficult problems. It is also necessary to develop infrastructure that facilitatesaccess to CNSs/CNs so that nurses can solve difficult problems. These findings also highlight the necessity of proactive participation in problem solving through collaborate inter-professional teams at the hospital.
