本研究の目的は,幼児期に糖尿病を発症し小児糖尿病キャンプに参加経験がある青年の「糖尿病をもちながら成長する体験」を明らかにし,長期的な成長発達の視点をもつ看護援助への示唆を得ることである。2～5歳で1型糖尿病を発症し,現在19～24歳の青年5名を対象に,発症時期から現在までの時間経過に沿って糖尿病や療養行動に関連した生活のあり様について自由な語りを得た。それぞれの時期の体験から,糖尿病をもちながら成長する体験,小児糖尿病キャンプの体験,家族のサポート,糖尿病になったこと,について抽出した。その結果,以下が明らかとなった。糖尿病をもちながら成長する体験は,糖尿病をパーソナリティーの一部として取り込み成長していく体験であり,青年期となった現在【時折,制約のある自分をもどかしく感じながらも,糖尿病のある自分はふつうで,よりよい人生】と肯定的にとらえていた。小児糖尿病キャンプは,糖尿病である自分がふつうに生活していくために有用な技術や情報を得た体験,同じ病気をもつ一生の友達を得た体験であった。また,家族は年少の時に生じた困難を子どもに代わって受け止め,自分の気付かないところで療養行動を支えてくれたと感じていた。これらの体験やサポートは,糖尿病をもちながら成長する体験が肯定的であることに影響を与えていた。以上より,幼児期に1型糖尿病を発症した子どもと家族に対し,糖尿病を子どものパーソナリティーの一部ととらえ,成長に沿って必要となる糖尿病のセルフケアが進むように支援していく重要性が示唆された。
The purposes of this study were to clarify the personal development with diabetes of adolescents with early-onset type 1 diabetes and investigate nursing intervention with long-term perspective on personal development for children with diabetes. Subjects were 5 adolescents who were diagnosed with type 1 diabetes in preschool and were 19-24 years old at the time of the study. Each subject had previously participated in diabetes camps. Data were collected by narrative interviews during which the adolescents described their experiences of living with diabetes. Interview data were analyzed by focusing on personal development with diabetes, experiences at diabetes camp, family support and the meaning of diabetes. The following results were obtained: Personal development with diabetes was the process of incorporating their experiences with diabetes into their personality. The adolescents indicated that for them, diabetes has a positive meaning. The meaning of diabetes was "sometimes get nervous about restriction for diabetes, but living with diabetes is normal, and life is better than it was without diabetes." Their experiences at diabetes camp provided them with useful skills and information for personal development with diabetes. They felt that family members avoided discussing difficult situations with the children with diabetes, and supported them with unfamiliar aspects of self-care behavior. Experiences at diabetes camp and family support allowed the adolescents to develop a positive meaning of diabetes. These findings suggest that nursing intervention should promote the understanding that diabetes is part of the personality of children diagnosed with early-onset type 1 diabetes and nurses should support the children's self care according to normal personal development.
