認知症高齢者は認知機能の低下から日常生活に支障をきたしやすく,多職種で連携して適切なケアを行う必要がある。しかし,優れたケアの知識は個人の暗黙的知識となっており,他者と共有されにくい。そこで,認知症ケアの知識を共有し活用しやすくするため,知識工学の手法を用いて,生活障害とケアに関する知識を構造化した。本研究は,日常生活のケアの中でも難易度が高いと考えられる排泄に関するものである。厚生労働省の研究班による調査報告書から,排泄に関する生活障害の具体像と奏功したケアの知識を取り出し,構造化を行った。その結果,これまで明示されていなかった生活障害の原因やケアの意図といった暗黙的な知識が明示された。また,一つ一つの生活障害に応じたケアを参照できるようになり,ケア提供者は認知症高齢者がどのような困難に直面して排泄が遂行できなくなっているのかを理解しやすくなった。これにより,ケア提供者は認知症高齢者のできないことに着目するのではなく,本人ができる部分を見極め,本人ができるように必要な部分を補うというケアが実現できるようになった。また,多職種間で暗黙知も含めた知識を共有できるようになった。以上のことから,本研究は,パーソンセンタードケアの理念に基づき,認知症高齢者の自律と自立を尊重する個別性に応じたケアの実践に寄与すると考える。
Older people with dementia are more likely to experience difficulties in their daily lives due to cognitive decline. They need appropriate care in collaboration with interprofessional. However, because knowledge of superior care is tacit knowledge of individuals, it is not easy to share with others. Therefore, to facilitate the sharing and utilization of knowledge of dementia care, we used knowledge engineering methods to structure daily living difficulties and care knowledge. This study is related to excretion, considered one of the most challenging aspects of daily living care. From the survey report by a research group of the Ministry of Health, Labor, and Welfare of Japan, we structured the knowledge about daily living difficulties and successful care regarding excretion. Consequently, tacit knowledge, such as the causes of daily living difficulties and intention of care, which had not been explicitly indicated in the survey report, were clarified. In addition, carers can now refer to the care for each of the daily living difficulties, making it easier for them to understand what kind of difficulties older people with dementia face that prevent them from performing excretion. Carers can provide care that does not focus on what older people with dementia cannot do but assesses what they can and provides care where they need it. In addition, knowledge, including tacit knowledge, can now be shared among interprofessional. Based on the above, we believe this study will contribute to the practice of individualized care that respects the autonomy and independence of older people with dementia, based on the philosophy of Person-Centred-Care.
