内容
目的:本研究の目的は,同種造血幹細胞移植(allo-HSCT)を受けた造血器腫瘍サバイバーが職場復帰後に就労を継続していく体験を明らかにすることである。方法:一施設の血液内科外来で,allo-HSCT後に職場復帰している造血器腫瘍サバイバー8名を対象に就労に関する思いや治療に伴う影響,職場との関わり,支え等について半構造化面接を行い,質的帰納的分析を行った。結果:対象者は子どもをもつ男性8名で,うち2名が自営業者であった。移植時の平均年齢は45.6歳で,全員が何らかの移植後合併症を発症した経験を有した。カテゴリは,【移植後の脆弱化した心身で働き続けるために負荷を避けた無理のない働き方の試み】【逃れられない治療の影響を抱えながら働くにはどうすべきか考えた行動】【長期の病気休暇あけの自分に対する職場での視線の懸念】【発病前のように働けない自分への否定的感情】【造血器腫瘍から職場復帰までの険しい道のりを経た自身の成長の実感】の5つに集約された。考察:allo-HSCT後サバイバーは長期の休職を経て,再発リスクを抱えた上に定期的な通院で合併症・後遺症に対する治療や予防を継続しながら,これまで培ってきた勤務経験を活かしつつ,徐々に業務量や内容を拡大し,安全に就労を継続する必要があることから,初発時から専門家と職場の協働した両立支援が不可欠であると示唆された。
OBJECTIVE: The objective of this study was to investigate the experiences of hematopoietic tumor survivors who underwent allogeneic hematopoietic stem cell transplantation (allo-HSCT) and have returned to work. METHODS: Semi-structured interviews were conducted to investigate the experiences of survivors who returned to work after undergoing allo-HSCT in a hematology outpatient department. Furthermore, a qualitative inductive analysis was performed. RESULTS: The subjects were eight men with children, two of whom were self-employed. The mean age at the time of transplantation was 45.6 years, and all participants had experienced a particular post-transplant complication. The study identified the following five categories: "Attempts to work in a reasonable way, avoiding the burden of continuing to work with a fragile post-transplant body and mind”, “Actions I have taken to consider what I should do to work while coping with the effects of treatment that I cannot escape”, “Concerns about how I would be looked at in the workplace after a long sick leave”, “Negative feelings toward myself for not being able to work as I did before the illness”, and “A sense of personal growth throughout the difficult journey from hematopoietic tumor to return to work”. DISCUSSION: allo-HSCT survivors are recommended to return to work safely after a prolonged absence while receiving ongoing treatment and preventing potential complications and sequelae through regular hospital visits. To achieve this, survivors should gradually increase their workload while utilizing their accumulated work experience. In addition, collaborative support from specialists and the workplace is essential from the onset of treatment.
